Attn: 5th Grade Parents

A Yearbook Permission Slip was sent home a couple of weeks ago, and we're trying to collect these slips by Tuesday, November 1st.

If you haven't submitted your permission slip, please do so as soon as possible.  We are asking for you to return the slip, your child's signature, and your child's baby photo to his/her homeroom teacher in an envelope marked "5th Grade Yearbook".

If you need an additional permission slip, the Yearbook Committee will be available at the Halloween Dance on Saturday night and/or we can email you an additional permission slip.

Please email your request or any questions you may have to:  pgyearbook12@gmail.com.

Thank you for your help, the 2012 Yearbook Committee.
